Please make certain to bring any important medical records from your previous provider (if you are a new patient) or other providers seen since your last visit here, especially important medical records from other health care providers, hospital notes , urgent care or Emergency Room visit notes, your most current insurance information, photo identification, a list of all previous immunizations or shot records and your medications in their original bottles so that we can update your medical records. For child wellness examinations we will also need any school forms including sports participation examinations that you wish to have completed with you at the time of the office visit. If you see another medical provider for your annual pap, pelvic, breast exam and or bone density and mammogram results, please bring that information with you or have it faxed to our office at 828-728-0868.
If this is to be a laboratory visit: to properly evaluate your health, we will need for you to be fasting for your laboratory studies. That means that you should not eat or drink after 8 PM the night before the examination. If you have afternoon fasting lab visit, please eat a light breakfast and skip lunch and not eat or drink before your afternoon lab visit. In either case it is ok to drink black coffee or preferably water up until the time of the lab testing. If this is just for a PT (Protime) test to evaluate warfarin dose, you do not need to fast but do not take your warfarin on the morning of the test.
If this is for a CDL/DOT Certification examination: In order to follow the guidelines to grant approval for CDL/DOT certification is necessary for you to bring your legible driver’s license and any other additional photo identification, any and all of your medications in their original bottles. If you have sleep apnea or use CPAP, please bring a report of your compliance/use over the last 6 months. If you are diabetic we need a hemoglobin A1c under 8% within the last 6 months. If you do not have this, we can perform this test for an additional $25 fee.
If you are over 35 years of age and a smoker, we need you to bring with you or we can perform a breathing test/spirometry with an additional cost of $25. If that test is not within an approved range we will need to refer you for a chest x ray. If you have had a heart attack, stent or a history of angina or any cardiac condition we will need a note from your cardiologist or primary care provider and a stress test that has been performed within the last 2 years. We can perform a stress test here if necessary, but this must be arranged in advance and there is an additional fee. There is an additional charge of $25 if our office is required to obtain this information for you. Part of being certified as a skilled driver is documentation and following directions and failure to bring required items with you may result in failure of certification.
